NMC Rules After a Malaysia MBBS: FMGE and Practising in India (2026)
If your child studies medicine in Malaysia, can they practise in India afterwards? Yes - through a published regulatory path. But the rules set hard conditions on the course itself, and choosing wrong is expensive. Here are the National Medical Commission's own requirements, checked 31 October 2026.
The conditions the NMC sets on your foreign course.
From the NMC's Foreign Medical Graduate Licentiate (FMGL) Regulations, 2021 - which apply to anyone who joined a foreign medical course after 18 November 2021:
- Minimum 54 months of course duration.
- Medium of instruction: English.
- A 12-month internship at the same foreign institution that awarded the degree, with hands-on training across named clinical subjects (General Medicine, General Surgery, Obstetrics and Gynaecology, Paediatrics, Psychiatry, Orthopaedics, Emergency/Casualty and more). No part of the course or internship may be done in India or any third country.
- The course and internship must be commensurate with the Indian MBBS under the Graduate Medical Education Regulations.
- You must be eligible for registration with the professional regulator of the country that awards your degree - at par with that country's own citizens. For Malaysia, that means the Malaysian Medical Council register.
- The full course must be completed within ten years of joining.
- After returning: qualify the screening test (currently the FMGE, conducted by NBEMS; the regulations name the National Exit Test or other mandated test), then a further 12-month internship in India through a State Medical Council under the CRMI Regulations, 2021, before permanent registration.
One clarification from the NMC's own page: the NMC does not process individual equivalence applications for foreign degrees - the examination and registration route above is the only mechanism.
What this means for a Malaysian MBBS choice.
Malaysia's established five-year English-medium programmes fit the 54-month and language conditions on paper. The conditions that need active checking for any specific university are the internship structure (twelve months at the same institution, with the named clinical rotations) and registrability with the Malaysian Medical Council at par with citizens. These are checkable facts about a specific programme - and worth confirming in writing before enrolment, not assumed from a brochure.
The honest picture.
The India-practice route is real and walkable, and thousands of foreign-trained doctors register every year. The hurdles are equally real: the FMGE is a genuine exam with a meaningful failure rate, the internship conditions eliminate shortcuts, and a course that does not map to the FMGL conditions cannot be fixed afterwards. Families who verify the fit before applying keep every option open.
